Description

MAHS is an acronym for MORE ALLIED HEALTH SERVICES. The program supplies Dietitians and Podiatrists to GPs surgeries on a roster basis. The Dietitians and Podiatrists support GPs in the management of people living with diabetes by providing individual counselling to the patients.

Division GP Support – How we can help you ?

The dietitians and podiatrists conduct one to two sessions at each GPs surgery in a four-week roster. The sessions are 3½ hours. Extra sessions are provided to surgeries on a needs basis.

Individual appointments are made for people with diabetes. Forty-five minutes is allocated to a new patient and thirty minutes for a review patient.

Surgery staff / practice nurses are instrumental in the administration of the MAHS program in the individual surgeries.

The Care Planning Process works very well with the MAHS programme. The dietitian and podiatrist have a clear understanding of the process, and are easily accessed for collaboration, appointments, reviews, and ongoing patient management.

The roster provides allocated sessions for Case Conferencing every 12 weeks, with both the podiatrist and dietitian attending the surgery for this purpose. As with the Care Planning process, the dietitians and podiatrists are conversant with the Case Conferencing process. Where requested by the GP, the Dietitians and Podiatrists can make suggestions to the GPs as to patients who would benefit from a Case Conference.

MAHS Diabetes Resource Folder

A MAHS Resource Folder is provided to all GPs participating in the MAHS Program. The folder includes information on diagnostic screening, a diabetes management flow chart, PIP and SIP information integrated into the management flow chart, a local diabetes service directory, and patient handouts.

Practice Support

Administrative support provided to administrative staff to customise the MAHS Program to individual practices’ needs. Practice support visits available to GPs and staff in relation to care planning processes within the practice.
