Chronic and Complex Disease Prevention (Lifestyle Modification Program)

The Shoalhaven Wellness Centre (Nowra) and Healthfit (Milton) are able to offer a government funded, Lifestyle Modification Program (LMP) which aims to reduce the risk of individuals developing Type 2 Diabetes.

The program targets individuals aged 40 to 49 years of age who are identified as being at high risk of developing Type 2 Diabetes using the AUSDRISK assessment tool.

Group education sessions over a 6 month period will focus on physical activity and healthy eating. Participants will be engaged in activities that provide them with tools to implement long term behavioral changes and take control of their own health.

More information on the LMP program can be found in the Wellness Centre Brochure under GP resources or by contacting [julia at sdgp dot org dot au] at the Division.
