Description

In reponse to diabetes reaching epidemic proportions in Australia, the Shoalhaven Division of General Practice has implemented a component of the Australian Better Health Initiative (ABHI).  Specifically this program is to promote solutions to primary care integration between general practice and other local health providers that will assist in more 'seamless' patient care.  The program also aims to improve the knowledge of patient's, GP's and other health care providers about diabetes and enhance our current work with the Koori population and Torres Strait Islander people.

GP Resources: 

The program provides GP's with a comprehensive resource of diabetes services and health providers in the region.  Where applicable, provider information includes estimated waiting periods and Argus subscribers.  A hard copy of this resource is available, however links to completed sections are below:

Phone the Division to obtain information about diabetes related services including a copy of the local service directory and to receive fact sheets and brochures from Diabetes NSW, Vision Australia, Heart Foundation, NSW Health and Kidney Australia.
