Description:

The Aged Care Access Initiative aims to improve access to primary care (GP and Allied Health Services) for residents of aged care facilities (ACF).

 Activities include:

  • Kitchen Garden project involving a local Residential Aged Care Facility and closely located primary school.
  • A exercise physiology program for residents in participating local Residential Aged Care Facilities.

 

Division GP Support

Education

Education provided for general practice personnel and Residential Aged Care Facility staff.

Access to Specialist Services

Formal arrangements with specialists in geriatrics, psychiatry and palliative care will be established to assist GP’s in their management of complex clinical cases

Liaison with ACF's

By maintaining a close working relationship with ACF’s it will be possible to enable GP’s to improve their efficiency of access to ACF’s.
